Thread: Image upload fails

I am getting a lot of complaints from users who can't upload images.
When they try to upload a photo the file seems to be uploaded but when it's completed they don't see the new image and the upload page just refreshes itself.
There is no error message nor can I see anything in the error log.
Ideas?
